Registration Form for Cape Fear Isshin-Ryu Summer Camp Program 2024

I agree to waive any and all claims against persons connected with Cape Fear Isshin-Ryu Incorporated.
As we are aware, very young children are prone to mishaps. Although we will give every effort to have adequate supervision, occasionally accidents will happen. In the instance that an emergency arises we would consider this form your specific permission to transport and have your child treated by a professional caregiver/physician, etc. This form also serves as specific permission to pick your child up from their respective school and transport them back to our dojo for our karate after school program. Cape Fear Isshin-Ryu reserves the right to remove any child from the program. This is for the safety and well being of all students and staff.

  • *BY SUBMITTING THIS FORM, I UNDERSTAND AND AGREE TO THE FOLLOWING:
  • *IN ORDER TO RECEIVE THE LOWER WEEKLY RATE OF $170 PER WEEK I AGREE TO PAY THE NON-REFUNDABLE $95 REGISTRATION FEE AT TIME OF REGISTRATION. I ALSO UNDERSTAND THAT IF I REGISTER FOR THE ENTIRE SUMMER, I AM ABLE TO USE ONE WEEK OF VACATION DURING THE SUMMER PROGRAM - BUT I MUST SUBMIT MY VACATION WEEK REQUEST IN WRITING A MINIMUM OF 7 DAYS PRIOR TO THE WEEK REQUESTED IN ORDER TO USE IT.
  • *I UNDERSTAND THAT I CAN ALSO CHOOSE TO PAY THE REGISTRATION FEE OF $95 AND CHOOSE SPECIFIC WEEKS AT TIME OF REGISTRATION TO QUALIFY FOR THE LOWER WEEKLY RATE OF $170 AND NOT BE REQUIRED TO PAY THE PER WEEK DEPOSIT TO RESERVE MY SPOTS. HOWEVER IF I CHOOSE NOT TO SEND A CAMPER FOR THE RESERVED WEEKS WITHOUT A WRITTEN NOTICE TWO WEEKS PRIOR TO THE FIRST DAY OF THE REGISTERED WEEK OF CAMP, I WILL BE CHARGED A $25 CANCELLATION FEE FOR THAT WEEK.
  • *I UNDERSTAND THAT I CAN CHOOSE TO REGISTER FOR SPECIFIC WEEKS AND NOT PAY THE REGISTRATION FEE OF $95. HOWEVER, I WILL NEED TO PUT A NON-REFUNDABLE $25 DEPOSIT TOWARDS EACH WEEK I RESERVE. THIS $25 WILL BE APPLIED TO THE WEEKLY RATE OF $185 FOR SINGLE WEEK CAMPS. IF I CHOOSE TO NOT SEND A CAMPER FOR THE RESERVED WEEKS, I UNDERSTAND THAT I WILL FORFEIT MY $25 DEPOSIT.

  • Summer Camp Registration Options:

  • I would like to register my child for the whole summer (excluding one vacation week) and pay the $95 non-refundable registration fee:
  • I would like to register my child for the multiple weeks marked below and pay the $95 non-refundable registration fee to receive the lower weekly rate of $170.00. I understand that if I register my child for a specific week and choose not to send them to camp I will be charged a $25 cancellation fee unless a written cancellation request is submitted two weeks prior to the first day of the registered week of camp.
  • I would like to register my child for the following weeks of summer camp. I understand that I will pay the single camp weekly rate of $185 and need to put a deposit of $25 per week to hold my spot. This deposit will be applied to the weekly $185 tuition cost. If I choose not to send my camper on the reserved week, I understand that I will lose my $25 deposit.

  • Transportation Permission:


  • During the summer camp program it may be necessary to transport a camper/student to an event or on a field trip via Cape Fear Isshin-Ryu Incorporated vehicles or a staff passenger vehicle. Therefore, we must have your permission to transport your child in such vehicles as circumstances warrant. Your signature below authorizes us to do so as required. "I authorize my child/children to be transported to and from camp activities via Cape Fear Isshin-Ryu Incorporated vehicles or a staff passenger vehicle." By signing this, I am giving my permission for my child/children to participate and go on any prearranged field trips.
